Many insurance plans offer massage as part of your benefits. Silver Creek Cottage Massage will be happy to do a FREE benefits check for you to see if your plan qualifies for treatment massage. Below is a list of insurance plans we accept.

Insurance typically only covers massage for treatment purposes only, and most times you will need a prescription
from your doctor. However, if you have been suffering from chronic pain in areas of your body or have recently
suffered an injury (work, auto accident, fall, etc.), in most cases massage will be covered depending on your plan.

Please contact Silver Creek Cottage Massage to discuss your insurance benefits and provide your date of birth, id number, group number and customer service phone number for providers listed on the back of your insurance card. We will check your insurance prior to your first massage appointment. Note: Even If your insurance plan is listed above, it does not mean that your particular plan provides for massage coverage. Silver Creek Cottage Massage will contact your insurance company prior to your appointment to confirm your benefits and determine any charges you may be responsible for (i.e. co-insurance, co-pays, etc.). If for any reason, your insurance company denies the claim once submitted, you will be responsible for all charges. |
